Medication Safety: How We Keep It Simple and Consistent

Medication mistakes are one of the most preventable risks in home care, and usually come down to inconsistency rather than any single error - a missed dose here, a mistimed one there. The fix isn't complicated: consistency, clear records, and staff who are trained to follow the same process every time.

Every Pluto BV care worker follows the same basic routine for medication support: confirm what's due against the client's medication list, prompt or assist as agreed with the client and their GP or pharmacist, and record what was given and when - every visit, without exception.

We deliberately don't ask staff to make judgment calls about dosages or changes to medication. Anything outside the agreed plan gets raised with the client's family or healthcare provider rather than handled informally in the moment.
